After surgery or hospitalization, not all patients are not ready to return to their homes right away. To aid in the transition from the hospital to a WellBridge facility, a liaison meets with the patients and their families at the hospital to answer questions and works with the hospital to make the transition as smooth as possible. Following a stay at WellBridge, Care Transition staff members can also help arrange any services needed at home, such as equipment or health care.
